CANCELLATIONS:
We understand circumstances may arise that make it impossible for you to keep a scheduled appointment. If you need to cancel or reschedule your appointment, please call 24 business hours before your scheduled appointment time. If we are not available, please leave a message on our voicemail. Any cancellations made less than 24 business hours in advance will be charged a fee. Should this happen in the future, please call our office as soon as you know the appointment will need to be rescheduled. The earlier you let us know, the more likely we can offer your scheduled appointment time to another patient in need of care.
LATE ARRIVALS:
If you will be late to your appointment, please contact our office to inform us. We do allow a 10-minute grace period, but if you arrive after our grace period, the provider will be unable to see you and we will need to reschedule your appointment. A missed appointment fee will apply.
NO SHOWS:
If a patient fails to present for a scheduled appointment or arrives so late for the appointment that they are unable to be seen a “Missed Appointment Fee” for the cost of the appointment will be applied to the account & will need to be paid before the next appointment.
New patients who miss their appointment will be charged the full cost of a new patient appointment - $318.00.
If you have more than one no show, you may be asked to keep a deposit on file in order to book an appointment.
MEDICAL BENEFITS: Health insurance plans vary greatly even within the same insurance company, so it’s important to be familiar with your own health insurance benefits before seeking care. Please see the New Patient tab for a list of questions you should be asking your insurance company before scheduling an appointment. Knowing your insurance benefits is the responsibility of the patient.
ACCOUNT BALANCES: Patients are responsible for paying any and all medical expenses incurred at the clinic. Your health insurance will be billed using information provided by you at the time of service. If you do not have medical insurance, you will be responsible for the bill at the time of service. Monthly statements will be sent when there is a patient balance and payment is expected within 30 calendar days. Balances due over 90 days are required to be paid in full. Unpaid balances over 90 days from the date of the first statement sent will be sent to collections and appointments will not be scheduled until balance is paid or payment arrangement is established.
MASK POLICY:
Oregon no longer requires a mask to be worn in medical settings. Our clinic will only require a mask to be worn if you are experiencing respiratory symptoms - cough, cold, symptoms of COVID-19 or flu.
FRAGRANCES:
Due to patient and staff sensitivities, our office has a Fragrance-Free policy. Do not wear perfumes, colognes, essential oils and other fragrances when visiting our establishment. We appreciate your understanding in this matter.

We cannot bill Medicare or Medicare + Supplemental plans. Some Advantage plans do offer Naturopathic coverage. Please call your plan for more information.
