New Patients

NEW PATIENTS:

Please complete our intake paperwork below.  Once intake forms are complete and returned to the clinic (via secure email, US mail or by drop off) our office will contact you to schedule a new patient appointment. 

  • Medicare does not pay for Naturopathic services. We are unable to bill Medicare for your visits. We are able to bill some Advantage plans. Please check with your Advantage plan to see if our doctors are in network and if your plan has Naturopathic coverage. 


  • Our office is NOT currently taking new Kaiser / CHP or Humana patients, and we do not take UnitedHealthcare Retirement plans. 


  • Due to sensitivities of patients and staff, we ask that you kindly refrain from wearing perfumes, colognes, essential oils and any other scented items to your appointment. 


Before your appointment: 

Contact your insurance company and ask the following questions:

  • Does my policy cover services by a licensed naturopathic physician?
  • Is my physician in network with my plan? (You must ask about the physician by name, not as Hillsboro Naturopathic Clinic)
  • Do I need a referral or pre-authorization to see a naturopathic physician?
  • Do I have a maximum number or visits or a maximum dollar amount for naturopathic services per year?
  • Is there a deductible I need to meet for these services?
  • Do I have a copay?


For Acupuncture: Contact your insurance provider to ask if Acupuncture is a covered service on your plan & if a prior authorization is required. Please ask your insurance if a copay is required for both Office Visits and Acupuncture. 


We do offer time of service discounts for patients who do not use insurance benefits.
